Scheduling a new appointment

ATTENTION: All appointment requests will be contacted within 24 hours during business hours Monday- Friday 8:00am-4:00pm. Please indicate if you prefer to be contacted via phone otherwise you will receive an email reply.

Information needed to schedule an appointment includes baby’s date of birth, pediatrician, and insurance information including member ID.

We are now partnering with The Lactation Network. If you are breastfeeding and have BCBS PPO, Anthem PPO, Cigna PPO, UHC/UMR please go to https://go.lactationnetwork.com/ConnectionsOccupationalTherapy/CaraDaubenspeck to see if your visits may be fully covered by your insurance. Please complete this information at time of scheduling so we know how to bill.
